Social Worker – Adult Social Care Learning and Disabilities

Perm available – £38,220 – £40,777 Annually
We are seeking a committed and motivated Social Worker to join our Adult Social Care service within the People Care – Social Care and Education Professional Pathway. This is an excellent opportunity to make a meaningful difference by supporting adults with diverse and complex needs, using a strength-based and person-centred approach.
As a Social Worker in Adult Social Care, you will:
* Undertake Care Act assessments and reviews for adults with varying levels of need
* Work collaboratively with individuals, families, carers, and partner agencies
* Develop outcome-focused support plans that promote independence and safety
* Manage risk and complete safeguarding enquiries under Section 42 of the Care Act
* Participate in duty and act as a trusted assessor.
* Prepare and contribute evidence for Court of Protection applications with guidance from a Practice Supervisor.
Professional Development
* Newly qualified Social Workers will complete the Assessed and Supported Year in Employment (ASYE) within their first 12 months
* Opportunities for progression to Advanced Social Worker (Grade 11) through the Progression Pathway
* Ongoing supervision and professional support
Essential Requirements
* Degree or equivalent qualification in Social Work
* Registration with Social Work England
* Experience in adult social care, including assessments, reviews, and safeguarding
* Strong communication, analytical, and report-writing skills
* Knowledge of relevant legislation and strength-based practice
* Ability to work independently and collaboratively
