Job Description for Crisis Resolution Home Treatment Team Post
The post holder is responsible for assessing care needs and developing, implementing, and evaluating care programs for Adults and Older Adults with mental health issues referred to the Crisis Resolution Home Treatment Team.
Reporting to the Clinical Lead on a daily basis and ultimately accountable to the Matron, the post holder will be part of a multidisciplinary team, working closely with wards and primary health care professionals. They will also ensure standards of care are maintained within the service and promote an integrated, collaborative working environment.

Key Responsibilities
1. Provide holistic health and social care assessments, plan, implement, and evaluate interventions, and document care accurately.
2. Participate in initial client assessments to determine suitability for the service and liaise with referring agencies to meet client needs.
3. Engage in team and clinical meetings, contribute to care model discussions, and support research initiatives.
4. Maintain awareness of personal values and attitudes to ensure non-judgmental, objective client assessments.
5. Establish therapeutic relationships with clients and their families.
6. Ensure confidential, effective communication between care agencies, including case conferences and record-keeping.
7. Conduct clinical risk assessments and monitor clients' mental states to evaluate risks.
8. Identify and manage clients with behavioral issues, organizing joint visits if necessary.
9. Build strong links with relatives and carers, ensuring effective communication during hospital admissions and discharges, and informing relevant agencies for follow-up care.
10. Report breaches of professional conduct to the Line Manager or Supervisors as required.
11. Ensure policy compliance and development within the area.
12. Work flexibly across the Crisis Resolution Home Treatment Service spectrum.
13. Engage in clinical work appropriate to experience and skills, in consultation with supervisors and senior staff.
14. Contribute to service quality improvements and provide cover for crisis services as needed.
15. Participate in on-call arrangements for the service.
