Do you want to join the St Leonard's Hospice Team to expand our palliative care services within the local community?
We are recruiting two Wellbeing Coordinators to join the Hospice's Wellbeing & Outreach Team to extend our palliative care services to all demographics of the local population, ensuring our services meet community needs.
This is a full-time position working 37.5 hours per week during office hours, Monday to Friday, for a fixed period of 2 years.
Main duties of the job
The main duties will include:
* Create and maintain strong relationships with external organizations and community groups to raise awareness.
* Enhance the organization's understanding of the community we serve and incorporate community feedback.
* Help remove barriers to accessing palliative care services.
Our ideal candidates will:
* Have experience providing face-to-face support to individuals or families in care or support settings.
* Possess excellent interpersonal skills, with sensitivity and empathy.
* Be approachable, honest, and trustworthy.
* Ability to manage multiple priorities simultaneously within deadlines.
It is essential for this role to have a driving license and access to a personal vehicle, as it involves engaging with community groups.

Job responsibilities
To work within the Wellbeing & Outreach team and collaborate with internal teams to broaden the reach of our services, ensuring they meet the diverse needs of the community.
To understand and incorporate the community's voice and lived experiences into our service development.
Main duties and responsibilities
* Build and sustain relationships with external organizations and community groups to promote awareness.
* Engage with community and voluntary groups, faith organizations, and local stakeholders.
* Gather insights from the community and feedback from those with lived experience to inform organizational practices.
* Act as a service user advocate, providing information and support to address perceived barriers to care.
* Handle complex and sensitive information with empathy, escalating issues as necessary.
* Support referrals to clinical services.
* Support community groups like Wellbeing Hubs, bereavement groups, and disease-specific social events, including virtual support.
* Support volunteers within the team.
* Manage administrative tasks such as scheduling meetings and handling correspondence, prioritizing workload effectively.
