Responsibilities
* Clinical: ensure high standards of clinical care for the patients under your management and support others to do likewise; promote a food first approach to nutrition support and advise on the appropriate use of oral nutritional supplements in accordance with agreed guidance.
* Practice: act as a point of contact and source of expertise for internal and external enquiries within your scope of expertise/limitations; provide dietetic care to patients in a range of venues under indirect supervision of a registered Dietitian (community clinics, learning disability homes, nursing/residential homes, community hospital ward and domiciliary visits).
* Communication: communicate effectively with patients and carers to maximise understanding of the condition and the care plan; use skills of persuasion, motivation and explanation with a variety of patients.
* Collaboration: communicate effectively with all members of the integrated community teams, GPs, Consultants and other professions to ensure a collaborative approach to management of patient conditions; assess capacity, gain valid consent, and work within the legal framework with patients who lack capacity to consent to treatment; participate in clinical cover for community colleagues on annual, sick or study leave as required.
* Professional development and documentation: maintain own clinical professional development, keep abreast of evidence based practice, adhere to the British Dietetic Association code of ethics and professional conduct, maintain accurate documentation, and communicate assessment and treatment results to relevant disciplines (via SystmOne tasks, reports and letters).
* Leadership and service delivery: communicate and work collaboratively with MDT colleagues across the health economy to ensure a coordinated integrated service; maintain specialist equipment within the Dietetic Team and participate in appraisal and personal development programs; lead by action, promote the role of the Community Dietetic Assistant Practitioner, Dietitians and the importance of nutritional care.
* Organisational: manage a designated area of work as agreed with the team lead/registered Dietitian; plan and organise effectively for patient management and time use; collect appropriate data and statistics for department use; comply with organisational and departmental policies and procedures and participate in reviews as appropriate.
* Values and behaviours: demonstrate the organisation's CARE values (Compassion, Action, Respect, Everyone) and Signature Behaviours in care and service delivery.
Qualifications and Requirements
* Under indirect supervision of a registered Dietitian, provide dietetic care to patients in various settings and modalities as stated.
* Be actively involved in in-service training and maintain professional development as part of the role.
* Maintain documentation in line with legal and departmental requirements and communicate results to appropriate disciplines.
