As a Care Coordinator, you will be the central link between clients and the dedicated care teams, overseeing the planning, coordination, and ongoing management of care packages, ensuring outstanding service delivery and excellent communication at every stage. Duties
* Coordinating and scheduling care packages to ensure seamless service delivery
* Acting as a key point of contact for clients, families and care staff
* Supporting field staff and responding calmly to changes or challenges
* Maintaining accurate records and managing confidential information professionally
* Always delivering a high standard of customer service
