4Doncaster PCN is seeking an enthusiastic and motivated Care Coordinator to join our expanding Primary Care Network Team. The post holder will work as part of a multi-disciplinary workforce to improve patient care across out network of practices.


Main duties of the job

To work closely within a team to ensure thesmooth running of the Care Co-Ordinator Department. This will involve linkingwith GP surgeries and a range of Community Health Services

To support the Network Support Lead to delivereffective, co-ordinated care for patients and services offered by the PCN.

Set hours for this post

Thursday 08:00-18:00

Friday 08:00-18:00

About us

The 4Doncaster Primary Care Network was establishedby four practices situated in central Doncaster:

The Burns Medical Practice

The Kingthorne Group Practice

The Mount Group Practice

The St Vincent Practice

The network population is approximately 59000patients across the four practices. The practices have come together as anetwork in order to be able to respond in a flexible and responsive manner tochanges taking place in the NHS.

The establishment of the 4Doncaster network hasenabled the recruitment of additional staff, including Clinical Pharmacists,Pharmacy Technicians, Physiotherapists, Social Prescribers, Mental HealthPractitioners, Care Coordinators and Health & Wellbeing Coaches.


Job responsibilities

JOB DESCRIPTION AND PERSON SPECIFICATION

Job title: CARE COORDINATOR

Employed by and responsible to: Head of Workforce and Estates andHead of Finance and Performance at Kingthorne Group Practice.

Clinical Director: Dr MarkLeighton

Key Responsibilities:

Develop a thorough understanding of thevaccination programmes and targets that all practices within the PCN arerequired to meet.

Organise, plan and provide services to offercovid and flu vaccinations to the PCN and wider population either on site orremotely.

To support practices in coordinating thevaccination of care home and housebound residents (including the preparationand coordination of patient lists, planning visits and communicating withpatients)

Using GP systems to identify the cohort ofpatients who require specific vaccination, create and monitor recall systemsfor these vaccinations and ensure patients are offered appropriate vaccines.

Work in the reception area at your base toprovide a service to patients attending PCN clinics and deal with queries from patients,health professionals and colleagues in person or via email, telephone andSystmone tasks.

Use system reports alongside QoF, IIF &PCN DES requirements to proactively identify and work with specified groups ofpatients to deliver personalised care, to include the coordinating the bookingof identified patients into the practice clinics.

Take ownership and manage monthly rotas andappointments for Additional Roles Staff within the PCN also ensuring clinicsare fully utilised and take action to book all available appointment

Manage the network & Practice social mediaaccounts to promote all health related content and PCN services.

Record meeting minutes, capturing keydiscussions, decisions, and action points for timely distribution.

Build relationships with key staffin practices within the PCN. Attendrelevant meetings and integrate as part of the wider network team, providinginformation and feedback on relevant matters.

Acting as the first point of contact for theclinical team to provide support with general queries.

Actively be involved in Patient ParticipationGroups and feedback to the team on any actions.

Help people to manage their needs throughanswering queries, making, and managing appointments, and ensuring that peoplehave good quality written or verbal information to help them make choices abouttheir care.

Work alongside practice staff and PCN servicesto ensure cohesive care.

Where appropriate assist people to accessself-management health education courses, peer support or interventions that supportthem in their health and wellbeing. This may include attending any key meetingsand liaising with hospitals and community services.

Work with the GPs and other primary careprofessionals within the PCN to identify and manage a caseload of patients, andwhere required and as appropriate, refer people on to other healthprofessionals within the PCN.

Gather information and undertake enquires forthe PCN and the wider group as and when necessary.

Perform general administrative tasks as assignedby the PCN Manager to support PCN operations.

Be a proactive team player, be able to managetheir own time and workload and work under own initiative.

Manage patient records with attention toaccuracy, confidentiality, and data protection compliance.

Working with the Management team to schedule andparticipate outreach events and forums.

Support the coordination and delivery of the PCNled Multidisciplinary team (MDT) meetings.

Attend PCN and Locality Cancer meetings todevelop ways to increase patient attendance at screening appointments and rolethis out across the PCN.

Dealing with patient registrations as and whenrequested by practices.

Use system reports to send patient aimedfeedback questionnaires following appointments both at practice and PCN level.

File assigned pathology.

Produce a quarterly newsletter to be distributedto practices and on social media.

Action practice Smear prior notifications usingthe Care Identity dashboard.

To plan notice boards across sites as requiredensuring topics are seasonal and relevant.

To work alongside practices to promote andincrease NHS App utilisation.

Facilitate PCN staff social events.

You will be required to work across all PCNsites

Participate in any training programmeimplemented by the PCN as part of this employment.

Participation in an annual individualperformance review, including taking responsibility for maintaining a record ofown personal and/or professional development.

Person Specification


Experience

* Experience of administrative duties.
* Experience of dealing with sensitive/confidential information.
* Previous experience of SystmOne.
* Experience of working in Primary Care.

Qualifications

* Educated to GCSE level or equivalent
