Advanced Nurse / Paramedic Practitioners Band 7 Unscheduled Care, Isle of Skye
The post is suitable for a trainee Advanced Nurse/Paramedic Practitioner. Trainees will be employed on Annex 21 Terms and Conditions:
1. Up to 12 months prior to completion of training: 75 per cent of the pay band maximum of the fully qualified rate.
2. More than one but less than two years prior to completion of training: 70 per cent of the pay band maximum of the qualified rate of the qualified;
3. More than two but less than three years prior to completion of training: 65 per cent of the pay band maximum for the qualified rate.
4. More than three years from completion of training: 60 per cent of the pay band maximum of the fully qualified rate.
The Role
5. The Advanced Practitioner provides unscheduled care across the Isle of Skye, primarily focused on out of hours Primary Care during evening and weekends in North Skye. The role includes rotation to the Broadford Hospital A&E as service pressures allow.
6. The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during the in and out of hours period, covering seven days including public holidays. They will be responsible for the clinical decision making, assessment and treatment of patients in across the Primary, Community and Acute settings.
7. The Advanced Practitioner will work closely with the multidisciplinary team (MDT)/medical staff including the OOH Hub and Acute Services. Communicate when required to about any patient they have seen overnight/ at weekends who needs to be followed up by the GP / Acute Services during normal working hours.
8. The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced care. Medical telehealth/telephone support will be available when required from an identified source.

The successful candidate
You will be a Registered and experienced healthcare professional with a proven record of continuing professional development. Ideally in possession of MSc level qualifications in Advanced Practice.
You will be a team player, self-aware and ready to work amongst the community and the multidisciplinary team that cares for it.
