Community Nurse - EBBI Neighbourhood Team

The Evesham, Broadway, Bredon and Inkberrow (EBBI) NeighbourhoodTeam is a friendly and hardworking team who are looking for an enthusiastic nurseto join us at our base in Evesham.
The Neighbourhood team is looking to recruit a community nurse whois happy to work across our whole patch 37.5 hours per week and keen to jointhe team in supporting us to deliver high quality care.
We are looking for a passionate nurse who is excited by change andkeen to help shape a new way of delivering community nursing care. We work closely with our GPs and work hardat developing relationships with primary care services, drawing on the skillsof our MDT colleagues to manage complex patients and ensure we are responsive,and patient centred at all times.

Main duties of the job
Rolerequirements:
Working flexiblyacross the local area (the service operates 3 shifts within 7:30am 6:30pm,Mon-Sun)
The ability tomanage your own caseload and work within an MDT
The ability toproblem-solve and manage time effectively
Experience ofworking in a fast paced and varied environment
Your typical day will include home visits foradults of 18+ needing a range of nursing interventions. These include end oflife care, and pain/symptom management. Wound care: post-op wounds, leg ulcers,pressure ulcers and complex dressing regimes. Other skills required will be inIV therapy, injectable drugs, and holistic patient assessments. You will needto cover clinic patients and respond to unplanned and 2-hour request visitsalong with those from UCR.
Back at base, you will beinvolved in team meetings and care planning. You may also support the teachingof nurse students pre and post registration.

The post holder will:

This will be achieved through acombination of hands-on clinical skills and knowledge to treat, educate, andsupport patients on a designated caseload and by working with patients andtheir carers to assess, plan and implement appropriate packages of care.
Regularly take responsibility forthe caseload in the absence of the Senior Community Nurse.

Skills & Abilities
* Ability to challenge poor behaviour

Qualifications
* Registered Nurse (RN1)
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
* Accredited courses in palliative care, tissue viability, Diabetes
Experience
* Experience gained during Pre-Registration training
* 2 years post registration experience
* Community nursing experience
* Experience of management of caseloads
Knowledge
* Variety of evidence based clinical skills
* Confidence to teach student nurses, trainee nursing associates, patients, carers, and members of the multidisciplinary team
* Understanding of evidence-based nursing, multidisciplinary working, clinical governance
* Competence in male and suprapubic catheterisation
* Venepuncture and IV therapy
* Cannulation
* Management of syringe drivers
* Wound and Palliative care
Additional Criteria
* Able to meet the travel requirements of the role
