Diabetes Nurse Specialist Leading Transition & Education

The closing date is 05 August 2025

The post holder will develop the transition diabetes nurse role within the existing diabetes specialist nursing team. He/she will offer specialist clinical support to children and young people with diabetes, to develop services and improve the quality of care delivered in the various care settings. The post holder will exercise higher levels of judgement, discretion and decision making in clinical care demonstrated through the Trust competency framework. S/he will provide specialist expertise and advice demonstrating a sound understanding of the issues related to the identification, assessment, treatment and management of the client group. S/he will monitor and improve standards of care through, supervision of practice, clinical audit, evidence based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership, acting up in absence of team leader. S/he will contribute to clinical governance within the service by undertaking audit and research as part of the diabetes team.


Main duties of the job

* Works autonomously to support the management Transition and transition clinic
* In absence of the team leader manages the diabetes nursing team
* Provide a non-medical prescribing role, or working towards
* Manages own caseload and co-ordinates patient transition journey across divisions and organisational boundaries.
* Supports the diabetes lead nurse and manages the team in his/her absence
* Develops local policies, protocols and procedures in accordance with local and national standards in relation to specialist area of expertise particularly Transition
* To be the lead with diabetes research trials and studies
* To liaise with the clinical research facility
* Provides clinical supervision and mentorship to junior or new clinical nurse specialists/practitioners/nurses/therapists
* Maintains and updates patient and hospital staff education and training packages
* Promote people's equality, diversity and right
* to attend the DUK and National conferences and relevant workshops.
* Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of same.
* Continually monitor standards of care and participate in the improvement of care, through benchmarking, audit and research

Job responsibilities

* Works autonomously to support the management of the Transition caseload of patients whilst working as part of a multidisciplinary team
* Leads the Preparation for Transition nursing clinic
* In absence of the team leader manages the diabetes nursing team
* Provide a non-medical prescribing role, or working towards
* Facilitates good communication links between the child or young person with diabetes and carers/others involved in the management of the condition.
* Deals with calls/queries and communicate any plans to the child/young person with diabetes, their carer or other professionals as appropriate.
* Provides education and information to the child/young person with diabetes, their family and/or carers around diabetes related issues eg management of diabetes, lifestyle and recreation, safety, schools etcetera in order to prevent hospital admission
* Provides clinical advice, support and training to community health care professionals, education and social care teams.
* Provide training for schools and support with ongoing Educations and Health care plans for individual children
o Provide advice and help to families and initiate early help referral to social services as needed
o Enables the child or young person and carers choice and involvement in decisions about Transition care
o Co-ordinates patient care Transition pathway and liaises across organisational boundaries.
o Plans specialised programmes of transition care and provides specialised age-appropriate advice to the child or young person, relatives and carers contents
o Provides clinical advice in accordance with current evidence-based practice.
o Participates in nurse/consultant-led clinics and multidisciplinary team meetings.
o Participates in case conferences/MDTs in relation to future management or transition arrangements.
o Ensures the child or young person and their families are given appropriate verbal and written information regarding planned transition.
o Maintains accurate patient records and ensures all relevant information is documented in the patients medical and nursing records.

Managerial/Leadership:

* Manages own caseload and co-ordinates patient transition journey across divisions and organisational boundaries.
* Supports the diabetes lead nurse and manages the team in his/her absence
* Works in close collaboration with other health care professionals across organisational boundaries to ensure the service meets patients transition needs.
* Acts as a mentor to pre- and post-registration nurses, assisting in meeting their learning needs.
* Develops local policies, protocols and procedures in accordance with local and national standards in relation to specialist area of expertise particularly Transition
* Fulfils a non-medical prescribing role or working towards

Education and Development:

* Participates in training and education of the child or young person, their carer and health and social care professionals involved in the management of the condition to prepare for Transition
* Supports in the teaching and development of the child or young person and carers particularly education and social care carers
* Provides clinical supervision and mentorship to junior or new clinical nurse specialists/practitioners/nurses/therapists
* Maintains and updates hospital staff education and training packages
* Maintains and updates patient education and training packages
* Maintains own professional development, skills and competencies through participation in training and development activities.
* Maintains and updates evidence-based knowledge and skills.
* Fulfils all mandatory training requirements for the trust and assists others within the department to do so.
* Undertakes health promotion within the specialist area
* to attend the DUK and National conferences and relevant workshops.

Clinical Governance:

* Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of same.
* Continually monitor standards of care and participate in the improvement of care, through benchmarking, audit and research.
* Promote patient and public involvement activities in the specialist area, leading to service improvement.

Person Specification


Education and Training

* RSCN/RN (Child Branch)
* Experience delivering training and assessment
* Non-Medical prescriber or willing to work towards
* Post -Graduate Diploma in Children's Diabetes Care or working towards
* MSc or evidence of working towards


Experience

* Highly specialised knowledge and experience in diabetes care
* Highly specialised knowledge and experience with Transition in children and young people
* Caseload management
* Diabetes technology
* Delivering a Transition service


Knowledge

* Best practice within Diabetes Nursing
* Multi -disciplinary Team working
* Caseload organisation
* BPT and NPDA


Skills

* Participated in service development and implementation of change
* Able to present written information in a logical manner.
* Basic IT skills in word processing and spreadsheets
* Able to manage own caseload and work with minimal supervision
* Knowledge of professional and NHS issues
* Ability to develop effective interpersonal relationships with colleagues both in the health care setting and within education and social care
