Main duties of the job
Support the repeat prescribing process, ensuring safe, timely, and cost-effective supply of medicines.
Conduct medication reviews and reconciliations, particularly following hospital discharge.
Manage medication queries from patients, carers, and clinicians, escalating when appropriate.
Contribute to medicines optimisation projects, audits, and quality improvement initiatives.
Work closely with Clinical Pharmacists, GPs, and the wider team to support safe prescribing practices.
Maintain accurate and up-to-date medication records on the clinical system.
Help implement changes in prescribing guidance and respond to medicines safety alerts.
Promote patient understanding and adherence to prescribed treatments.

Job responsibilities
Prescribing and Medicines Optimisation
Support the repeat prescribingprocess, including synchronisation of medications and managing prescriptionqueries.
Perform medicinesreconciliation for patients post-discharge, ensuring accurate and safe updatesto medication records.
Review and process prescriptionrequests, ensuring safety, legality, and cost-effectiveness in accordance withclinical guidance.
Support medicines audits andswitches, including those required for ICB formulary or safety updates.
Answer medicines-relatedqueries from patients and carers, escalating where appropriate.
Provide advice on the safe andeffective use of medicines.
Support patients withmedication changes, including those linked to long-term condition reviews ordeprescribing initiatives.
Administrative and Quality Support
Support achievement of QoF, IIFand local medicines optimisation targets.
Participate in theimplementation of safety alerts and MHRA guidance.
Support the practice or PCNwith electronic prescribing, prescribing systems, and template development.
Teamwork and Development
Work collaboratively with GPs,pharmacists, nurses, and wider staff.
Attend training and supervisionsessions as required.
Contribute to qualityimprovement projects.
Maintain registration and keepup to date with CPD requirements.

Qualifications

* Complete PCPEP pathway
* Accredited checking technician or relevant post-qualification training
