An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.
X4 workers required for a 12 month period. 2 years post qualifying experience required. This will involve the ability to undertake face to face visits 5 days a week with 2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability.

Requirements:
* Assessing Needs: Conduct interviews and assessments to understand the needs of individuals and families.
* Developing Care Plans: Create and implement care plans to support clients in achieving better outcomes.
* Advocacy: Advocate for clients’ rights and access to necessary services.
* Collaboration: Work with other professionals in multidisciplinary teams to provide comprehensive support.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
