This role would suit either a newly qualified gp looking to develop their skills within a forward thinking and supportive practice or an experienced gp seeking to join a friendly multidisciplinary team the position is offered on a fixed term salaried contract with the possibility of extension. The gp role is designed to enhance patient care and improve access to primary care services as part of our multidisciplinary team the successful candidate will play a key role in delivering high quality comprehensive healthcare to our diverse patient population responsibilities will include managing patients with both acute and chronic conditions providing preventive healthcare services conducting medication reviews and contributing to personalised care planning the role will also involve working within our hub to triage econsults alongside another gp. Collaboration is central to this position with close working relationships across a skilled team of gps nurses pharmacists and allied healthcare professionals to ensure coordinated patient centred care. Key duties include managing routine and urgent appointments through face-to-face and telephone consultations as well as working within the hub alongside another gp to triage patients requesting consultations via econsult. Although home visits are primarily covered by our paramedic and nurse practitioner team there may occasionally be a requirement to undertake routine or urgent home visits administrative responsibilities include responding to clinical queries managing repeat prescriptions reviewing test results and preparing referral letters and other correspondence promptly and accurately the role also includes initiating investigations reviewing outcomes and referring patients to secondary care or other services where appropriate ensuring treatment plans align with nice guidance and chronic disease management protocols. The successful candidate will work collaboratively with health and social care professionals to support integrated patient care maintain accurate clinical records and contribute to high standards of clinical governance and service delivery. This is a six-session-per-week role across three full days working hours are either am to pm for regular clinics or am to pm when covering triage regular clinics consist of morning appointments and afternoon appointments. You will perform comprehensive clinical examinations and assessments, addressing both physiological and psychological needs, and developing care plans with timely referrals in accordance with practice, PCN, and local guidelines. Prescribing independently within your scope of practice, you will adhere to local or national prescribing guidelines while promoting health strategies that empower patients to adopt healthier lifestyles and embrace self-care. Responsibilities also include handling routine and emergency home visits, managing administrative tasks such as responding to queries, handling repeat prescriptions, reviewing patient test results, and preparing referral letters or correspondence in a timely manner. You will oversee clinical management, initiate investigations, review results, and refer patients to secondary care or other providers as appropriate, while developing treatment plans that align with disease management protocols and NICE guidelines. The role requires participation in Triage sessions and extended hours (8:30 am - 6:30 pm for clinical appointment sessions and 9:30 am - 7:30 pm for Triage sessions); clinical sessions will typically have 16 am appointments and 14 pm appointments.
