Job summary

The post holder is an Advanced Clinical Practitioner (ACP),delivering care to patients who are acutely unwell, housebound, recentlydischarged from hospital, or require clinical triage input. They will workwithin GPS Healthcare, a one-practice Primary Care Network (PCN) servingapproximately 40,000 patients in Solihull, as part of a multidisciplinary andmulti-agency team.

The ACP will be responsible for both urgent home visitingand clinical triage, managing a range of acute and long-term conditions andsupporting continuity of care in the community. They will deliver a highstandard of clinical assessment, diagnosis, treatment planning and evaluation,working autonomously within their professional scope of practice.

The post holder will demonstrate safe clinicaldecision-making and provide expert care, collaborating with the wider generalpractice team to ensure effective service delivery. The role supportsproactive, patient-centred care while contributing to the development ofclinical pathways and service improvement. Registration with the appropriateprofessional body and a recognised ACP qualification are essential.

Main duties of the job

Deliver a high standard of patient care to individuals intheir own homes, including those recently discharged from hospital, housebound,or with complex needs, using advanced autonomous clinical skills and broadclinical knowledge. Provide expert assessment, diagnosis, and management of arange of acute and long-term conditions, working both remotely andface-to-face.

Manage a daily clinical caseload involving urgent homevisits and triage, ensuring timely and appropriate care for patients across thePrimary Care Network (PCN), including prioritising same-day and routineappointments.

Respond to urgent and acute presentations as part of amultidisciplinary team (MDT), ensuring safe escalation and continuity of care.Actively participate in triaging patient contacts via telephone, digital, andonline systems, making autonomous clinical decisions within scope.

Support proactive care planning and early interventionthrough case identification, risk stratification, and the development ofpersonalised care plans. Where appropriate, engage in advanced care planningdiscussions with patients and families.

Collaborate closely with colleagues across the PCN and widerhealth and care system including community nursing, social care, mental health,and voluntary sector teams to ensure integrated, person-centred care delivery.

Contribute to service development and clinical qualityimprovement initiatives related to urgent care, triage and home visiting withinthe PCN.

Statement of Purpose

The post holder will provide services to thepractice population through home visiting services and clinical triage. TheAdvanced Clinical Practitioner will deliver care within the boundaries of theirrole and in accordance with specific practice guidelines and protocols. This role combinesface-to-face clinical assessment and management in patients homes with remotetriage for acute, urgent and routine clinical presentations.

They will demonstrate safe clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients within the general practice. The post holder will demonstratecritical thinking in the clinical decision-making process.

Main Duties and Responsibilities

Acute care i.e. participation in triage, providingsame day appointments, visiting patients at home.

Provide clinical assessments, diagnosis, andmanagement of patients in their own homes, including the elderly, housebound,or those recently discharged from hospital.

To prescribe safe, effective and appropriatemedication defined by current legislative framework and scope of practice.

Recognise and act on signs of deterioration,initiating appropriate treatment or escalation.

Liaise with GPs, community nurses, socialcare and community services to ensure continuity and integration of care.

Undertake clinical triage of same-day androutine appointment requests via telephone, digital platforms and onlineconsultations.

Prioritise triaged cases and allocate to themost appropriate clinician or service, or manage independently where withinscope.

Identify urgent/emergency presentations andact according to local safeguarding and escalation protocols.

Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based

treatment methods into a management plan.Refer when appropriate.

Work with other health care professionals toidentify, diagnose, monitor, manage and treat patients at risk of or withlong-term conditions, including on-drug-based treatment methods using amanagement plan.

Assess and refer patients presenting withmental health needs in accordance with the NationalFramework (NSF) for Mental Health.

Recognise adult and childsafeguarding situations and report appropriately.

Provide information and advice on prescribedor over-the-counter medication on medication regimens, side-effects andinteractions.

Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.

Support patients to adopt health promotionstrategies that promote healthy lifestyles and apply principle of self-care.

Participate in clinical audits, servicedevelopment, and quality improvement projects.

Maintain professional registration (e.g.,NMC, HCPC) and independent prescribing qualification.

Managinginformation

Usetechnology and appropriate software as an aid to management in planning,implementation and monitoring of care, presenting and communicatinginformation.

Engagein regular clinical supervision and peer review.
