Care Navigator - Primary Care (Merseyside)
Connaught Resourcing is supporting a valued client in the Merseyside area with the recruitment of Care Navigators for ongoing temporary assignments.
About the Role
As a Care Navigator, you will be the first point of contact for patients, carers, and healthcare professionals – providing compassionate, efficient, and professional support over the phone.
Key Responsibilities
* Handle incoming calls for GP practices, ensuring patients are directed to the most appropriate clinician or service.
* Escalate emergency calls promptly to ambulance services.
* Support the wider primary care team with follow‑ups and coordination.
* Accurately record patient information using the EMIS Web system.
* Liaise with external services and follow clinical instructions.
* Maintain confidentiality and uphold a patient‑first approach.
* Navigate local referral pathways to ensure holistic care.
What We're Looking For
* Strong verbal and written communication skills.
* Ability to work independently and as part of a team.
* Proficiency in Microsoft Office and EMIS Web (essential).
* Experience in a primary care setting and call handling.
* Empathy, patience, and professionalism in every interaction.
