What you will do:
* Deliver proactive street outreach, identifying and engaging rough sleepers, including those who have disengaged from services.
* Undertake comprehensive assessments, risk evaluations, and personalised recovery/support plans for each service user.
* Manage a caseload, coordinating care pathways including housing, substance misuse treatment, and mental health services.
* Support service users into emergency, temporary, and long-term accommodation, including resettlement planning.
Person Specification:
Qualifications:
* NVQ Level 3 (or equivalent) in Health & Social Care, Substance Misuse, Housing, or a related discipline, or relevant experience.
Registration & Compliance:
* Enhanced DBS clearance.
* Knowledge of safeguarding adults and children frameworks.
* Understanding of relevant legislation including the Housing Act, Care Act 2014, and GDPR.
Experience:
* Experience working with vulnerable adults within homelessness, substance misuse, mental health, or criminal justice settings.
* Proven ability to support individuals with multiple and complex needs.
* Experience of multi-agency working across statutory and voluntary services.
* Experience delivering person-centred, recovery-oriented support in community or outreach environments.
