As a Social Worker in one of our adult localities teams, you will be a relatable practitioner responding to situations with strengths‑based practice at the heart of your work. You will develop creative and imaginative solutions to help people achieve positive outcomes which enable them to maintain their independence and live happier, healthier lives. Here you will help individuals, families, carers, and communities to make the right decisions for a safer, more independent future and progress through our training programmes.
Key Responsibilities
- Produce high‑quality assessments and undertake preventative work and risk assessments utilising community resources and services such as reablement and telehealth care.
- Collaborate with various agencies, patients and carers to promote independence in a strengths‑based approach.
- Interpret case law and legislation, including the Care Act and the Mental Capacity Act, and commission and arrange appropriate services.
- Construct value‑for‑money care packages and coordinate service delivery.
Qualifications
- Recognised social work qualification and registration with Social Work England or a pending registration.
- Relevant experience working with adults.
- Knowledge of the Care Act, Mental Capacity Act, local and national policy issues, equal opportunity, and anti‑oppressive practice.
- Strong communication skills and a flexible, creative practice.
