Apply Save Clinical Pharmacist position full time within a GP Practice
This practice is seeking an enthusiastic Practice Clinical Pharmacist to deliver high-quality pharmacy-led services to our patients. This role involves working closely with our practice teams to improve prescribing practices, manage long-term conditions, and promote self-care. You'll lead medicines-related initiatives, mentor pharmacy staff, and drive operational efficiencies within the practice.
Key Responsibilities:
1. Manage long-term conditions and perform structured medication reviews
2. Provide telephone clinics and address prescription queries
3. Lead prescribing audits and contribute to Quality and Outcomes Framework (QoF) initiatives
4. Develop relationships with local community pharmacies and PCN pharmacy teams
5. Deliver training for GPs and prescribers and promote safe prescribing practices
6. Support care homes in improving medicines safety and management
Essential Requirements:
7. GPhC registered pharmacist with a degree or equivalent in pharmacy
Desirable:
8. Experience in primary care and clinical diploma
9. Independent prescribing qualification
