* Provide telephone, online and face-to-face consultations across our linked Leicestershire care homes and to our home-bound patients
* Lead on care planning, reviews, and proactive management for vulnerable patients

Job responsibilities
Clinical
Provide clinical care and examinations to frail patientsand patients within the practices associated care homes, including clinicaladministration as required.
Conductcomprehensive assessments of patient frailty levels and develop individualisedcare plans addressing frailty concerns, collaborating with healthcareprofessionals to implement personalised interventions.
Prescribetreatment including medications as a Non-medical Prescriber.
Clinical Governance
Contributeto Clinical Governance- reporting any Learning events, Significant Events,Complaints, or safeguarding concerns.
Conductregular audit of practices and protocols to ensure compliance, negotiatingchanges and introducing service improvements as required.
Support theRegistered Manager in compliance and targets for the practice.
Leadership and Collaboration
Provideclinical leadership, support and guidance to the practices associated carehomes and to other healthcare professionals internally involved in thepatients care.
Be a key team player and the go to person for clinicalissues for care home patients.
Coordinateinterdisciplinary teams, including nurses, caregivers, therapists, andadministrative staff, ensuring cohesive and efficient care delivery.
Activelyengage in MDT meetings.
Act as anambassador for the practice.
Professional Development
Maintainprofessional practice through continuing education, professional updating andwhen/where available, involvement with professional specialist groups.
Participate in own annual performance review, including takingresponsibility for maintaining a record of own personal and/or professionaldevelopment.
Adhere toNMC guidelines for continuing professional development and practice and ensurerevalidation criteria is met.
Familiarising yourself and adhering to all Company Policiesand Procedures
Person Specification
Qualifications
* Qualified/working towards appropriate Masters level qualification in Advanced Practice.
* To work within the regulatory requirements, codes and guidance of the Nursing and Midwifery Council.
Experience
* Ability to work autonomously with confidence to make decisions independently
* Ability to engage and communicate effectively with patients/stakeholders
* Critical thinking and negotiation skills
* Dealing with sensitive and complex information
* Competent in use of electronic systems e.g. EMIS, SystmOne, CRIS etc.
