As an HCSW you will play a pivotal role in co-ordinating support for people living with Parkinson’s disease and Epilepsy. You will have good communication skills as well as clinical skills and ability to manage own caseload whilst reporting to the Specialist nurses.
Band 3 HCSW work autonomously to provide a full range of support to Parkinson’s and Epilepsy patients who are house bound. Band 3 HCSW play an important role in providing seamless care and often liaise between more specialist services.
The Band 3 HCSW will also support the cardiac service to support the patients in managing their long term conditions, providing clinical assessment and regular reviews under the direction of the specialist Nurses.
Main duties of the job
Band 3 HCSW work closely with people who use our Services, their families, friends and carers and play an important contribution in how people experience our services. Band 3 HCSW are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide.
The band 3 HCSW will be responsible for :
Reviewing patient with Neurological or Cardiac disease and supporting them in their management of long term conditions
Keeping the people who using our services as safe as possible by recognising the limits of the HCSW role and working closely with Registered Nurses
Contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures when implementing care plans
Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans
Working in partnership with patients and their families and carers
Gaining consent and, as far as possible, involving people in all decision making
Signposting patients and carers to supportive services e.g. the courses provided by the Recovery and Wellbeing College or services provided by the local authority or the voluntary sector
Reassuring people by being professional, responsive, knowledgeable and confident
Escalating complaints or concerns effectively and quickly in line with the Trust policy

Detailed job description and main responsibilities
Band 3 HCSW will be expected to hold a caseload of patients allocated by the Specialist nurse of Parkinson and Epilepsy patients. The HCSW will also support the Specialist cardiac team in undertaking some duties supported by the Specialist nurses. Band 3 HCSW are expected to have previous experience of caring before coming into post and contribute to the effectiveness of services by:
1. Providing skilled, evidence based care which adheres to agreed policies and procedures
2. Working with patients and families and registered nurses to contribute to all stages of the care planning process including assessing risks and needs
3. Acting as patient advocates in the multi-disciplinary team
4. Working as part of a team implementing care plans or directives overseen by registered nurses
5. Contributing to creating and maintaining high performing teams by:
6. communicating well with all members of the team
7. understanding their role in the team and how they help the team achieve its’ objectives
8. reflecting on their own practice regularly
9. identifying how care could be improved
10. Working to the standards set out in the Code of Conduct for Healthcare Support Workers and Adult Social care Workers in England (Skills for Health 2013)
11. Safeguarding people by recognising and responding when an adult or child might be at risk from abuse but also recognising their own limits and asking for help and escalate concerns when necessary
12. Escalating safety concerns and by doing so acting as effective advocates for those who use our services
13. Being open and transparent about their own practice
14. Identifying areas where improvements in safety or quality can be made
15. Working closely with others to improve services
16. Maintaining accurate, legible, comprehensive records
17. Maintaining compliance with their mandatory training requirements
