Directorate: Adult Care, Housing and Public Health
Permanent, full time, 37 hours per week.
Subject to qualifying conditions, relocation support of up to £8,000 may be available.
Responsibilities
* Conduct assessments under the Care Act 2014 and Mental Capacity Act, and conduct Best Interests and Section 42 safeguarding enquiries.
* Co‑create tailored care and support plans that empower autonomy and independence.
* Collaborate with families, carers, health, housing, and voluntary sector partners to deliver holistic, integrated support.
* Monitor and review plans regularly to ensure they evolve with the individual's changing needs.
* Advocate for service users, championing their rights and choices.
* Maintain clear records and prepare professional reports for case conferences and reviews.
Qualifications
* Qualified (BA/MA/DIPSW) with valid Social Work England registration.
* Proven experience in adult social work, particularly supporting autistic adults and those with learning disabilities.
* Excellent knowledge of relevant legislation, including Care Act, Mental Capacity Act, Mental Health Act, and safeguarding frameworks.
* Strong assessment, analytical, planning, and risk management skills.
* Confident communicator with ability to build relationships across agencies.
* IT literacy and case management system experience.
