Job Responsibilities
* Act as a corporate parent for young people preparing to leave the care of the local authority.
* Provide qualitative assessments, ongoing support, advice, and guidance to promote the potential of care leavers to develop the skills necessary for independent living.
* Enhance the capacity for care leavers to realize their full potential and become successful in all aspects of their lives.
* Provide a targeted, outcome-focused service to young people who are legally defined as care leavers.
* Build and sustain relationships with young people to ensure their views are considered and that practice is centered on meeting their needs.
* Participate in assessment, preparation, and review of care leavers' pathway plans to equip them with emotional, social, and financial skills required for successful independent living.
* Coordinate the provision of services and encourage care leavers to utilize these services.
* Maintain effective communication with partner agencies and other professionals regarding the development of services and care leavers' progress and wellbeing.
* Keep full, accurate, and up-to-date records of contact with care leavers and services provided using the Council's electronic recording system.
* Provide a rapid and flexible response to support in crisis situations, which may include working outside of normal office hours and occasional weekends.
* Promote the Council's Equal Opportunities Policy in service delivery and treatment of others, considering young people's race, culture, disability, gender, and sexuality.
* Work towards local and national good practice guidance and respond to strategic and operational agendas in service development and delivery.
* Promote the best interests of children and young people by advocating for their rights and safeguarding them from harm.
* Participate in performance management, supervision, reviews, training, and development to improve skills and knowledge.
* Comply with the Council's Health and Safety Policy and associated safe working procedures and guidelines.
Person Specification
* Qualifications:
* Good general education to NVQ level 3.
* Desirable: NVQ 3 in Advice and Guidance or NVQ 3 in Health and Social Care.
* Knowledge & Skills:
* A clear understanding of the issues facing young people.
* Ability to establish and maintain effective working partnerships.
* Broad knowledge of IT applications and effective time management.
* Ability to work as part of a team and relate to a wide range of individuals and organizations.
* Desirable: Knowledge of education, training, and employment structures and trends; awareness of assessment methods for young people.
* Personal Competencies:
* Excellent communication skills.
* Other Requirements:
* Willingness and ability to travel throughout the Borough.
* Flexible and adaptable.
