The post holder will work within the Buckinghamshire Council Commissioning and Procurement Hub.
In line with Buckinghamshire Council’s Quality Assurance Framework the post holder will work with the wider team to support quality improvement of providers who deliver care in in residential and nursing care homes, homecare, supported living and day care. Working alongside Commissioners they will support provider managers and staff to ensure that care is delivered to meet required standards. This will require the development of active case management programmes for identified individuals with a variety of predetermined conditions and for organisations, where the identified issues are of a more systemic nature. The post holder will actively engage in Quality Surveillance Meetings and any other meetings that require specialist input, including, where the Councils Concerns and Serious Concerns processes are initiated. They will work with providers to support the development and ongoing review of improvement plans.
Working with a range of professionals from health, social care and the private, voluntary and independent sector, the post holder will enable high quality, appropriate care to be delivered and recorded in a timely manner.
The post holder will be able to demonstrate their professional skills and competence in leadership, training and coaching, assessment, care planning / co-ordination, risk assessment, medicines management, and care and treatment delivery.
Main duties of the job
Working with Buckinghamshire County Council you will oversee the Quality of care to residents in Care Homes across Buckinghamshire, supporting care home colleagues in their training and development needs to ensure the highest standard of care is provided to all residents at all times.
To work within the Council’s Integrated Commissioning Service to promote and support high quality care within care and support providers commissioned by Buckinghamshire Council. This includes working across the full range of care types including residential and nursing homes, home care, supported living, extra care and day services.

Essential criteria
1. Registered Nurse
2. Evidence of continuous professional development
3. Degree/ Diploma in Nursing Studies
Experience
Essential criteria
4. In depth experience of working within multi agency / multi-disciplinary team
5. Demonstrates broad experience within a variety of core areas e.g. care of the elderly, orthopaedics, neurology, palliative care, long term conditions
Desirable criteria
6. Minimum 3 years experience in community or inpatient setting
7. Experience of working in a community/in-patient setting for a minimum of 3 years post registration
8. Experience of assisting with implementing change management programmes e.g. service development
9. Experience of electronic clinical systems
