Salary: Grade 5, £17,562 - £19,340 per annum. £13.47 - £14.83 per hour (pro rata based on 37 hours per week)

Temporary until February 2026

Your role:

You will provide support to adults in a therapeutic setting and a reablement service, helping individuals maximize their skills, talents, and aspirations. You will carry out therapeutic activities as per assessment plans to promote independence and self-management, supporting adults who have been medically optimized to reach their full potential. You will assist your team leader in identifying appropriate service exits and current and future care needs, encouraging independence and self-management.

About you:

You should be able to work independently or as part of a team, providing positive outcomes for people with disabilities. Demonstrating values and compassion, you will support individuals to maximize their independence, take positive risks, and find solutions to maintain their autonomy.
