Overview
Band 3 Reablement Worker role based at Longmoor House on the Aintree Hospital site within Mersey Care’s community division. The post supports individuals to reach increased independence and return home and operates across a 24/7 shift pattern. The Reablement Worker will be part of the multidisciplinary team in a sub-acute intermediate care setting, reporting to a registered Nurse or Allied Health Professional.
Responsibilities
1. Deliver clinical and therapeutic care to patients as per care plans and assist nurses and Allied Health Professionals in patients’ treatment; undertake rehabilitation and reablement as directed by the multidisciplinary team plan.
2. Perform patient assessments (under supervision and after appropriate delegation from the registered practitioner) to identify goals and plan and deliver high standards of care.
3. Provide general and specific rehabilitation, reablement and personal care in line with treatment protocols; support patients to improve functional ability and mobility (e.g., bed mobility, transfers from chair to toilet, walking with a walking aid, and stairs if appropriate) and promote independent living.
4. Continue to develop and undertake a range of delegated tasks and work across professional disciplines; report to a registered Nurse or Allied Health Professional.
