Overview
The Denbighshire Learning Disability Community Health team at Betsi Cadwaladr University Health Board is seeking a Band 6 Community Learning Disability Nurse. The post holder will work as part of the Learning Disability team based at Denbighshire, coordinating interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, inpatient services, and other statutory agencies and the voluntary sector. Deliver evidence based, safe, effective interventions and clinical care following the nursing process. The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
Responsibilities
* Follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards.
* Communicate effectively with patients and families; be a visible and accessible member of the team, providing assistance, advice and support.
* Communicate and receive highly complex and sensitive information; analyse and interpret information and act appropriately where barriers to acceptance exist.
* Understand the CHC process and contribute to the development of pathways and the Maturity Matrix.
Qualifications
* RNLD Qualification
* Evidence of a degree level qualification or equivalent in line with NMC registration
* Evidence of postgraduate education and ongoing CPD
Desirable qualifications
* Management qualification and/or management experience
* Mentor/supervisor/assessor experience
Experience
* Significant experience within the clinical specialty or a relevant nursing role (essential).
* Experience of working in a multidisciplinary team setting (desirable).
* Experience/knowledge of senior nurse roles (desirable).
