We are delighted to announce exciting new opportunities to join the Community Asthma Service for Children and Young People within Mid and South Essex Community Collaborative (MSECC). The Mid and South Essex Community Collaborative (MSECC) brings together the three main providers for community services: Essex Partnership NHS Foundation Trust (EPUT), North East London NHS Foundation Trust (NELFT) and Provide Community Interest Company CIC in a Community Health Services Provider Collaborative. In order to drive this transformation we now require asthma nurses to provide expert advice and education to support diagnosis and management of children and young people with asthma within the community setting. The CCAN will work autonomously, manage a caseload of patients, work across various Mid and South Essex clinic sites, in collaboration with the Community Collaborative while working as part of the multi-disciplinary team, delivering individualised and personalised direct patient care. The CCAN is responsible for assessing care needs, developing, implementing and evaluating programmes of care without supervision, and will be required to teach other registered and non-registered nursing staff, educational professionals, parents/families/carers. In partnership with members of the multidisciplinary nursing team the CCAN will develop, implement and evaluate a seamless specialist service for children and young people with asthma, ensuring that service users receive the highest standard of clinical care implementing evidence-based nursing care.
Role and Responsibilities
* Manage a caseload of patients with asthma in the community setting, providing expert advice and education to support diagnosis and management for children and young people.
* Work autonomously and as part of the multi-disciplinary team across various Mid and South Essex clinic sites.
* Assess care needs, develop, implement and evaluate programmes of care without supervision.
* Teach other registered and non-registered nursing staff, educational professionals, parents/families and carers.
* Develop, implement and evaluate a seamless specialist service for children and young people with asthma in partnership with the multidisciplinary nursing team.
* Ensure service users receive the highest standard of clinical care using evidence-based nursing practices.
