Overview
The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team manager. The post holder will assist the District Nursing Team in delivering high‑quality community based nursing and social care to patients who have palliative care needs and wish to remain at home. The post holder works collaboratively with the Intermediate Tier Services in supporting patients and carers, providing personal, social, psychological and spiritual care, and regularly works without direct supervision in a patient’s home.
