District Nurse/Caseload Manager
NELFT North East London Foundation Trust invites applications for the role of District Nurse/Caseload Manager (Band 6) within Integrated Care Team (ICT North), Chingford, Waltham Forest.
Job Summary
The District Nurse/Caseload Manager is responsible for clinical leadership, coordination, and management of a community nursing caseload. The post holder delivers evidence‑based nursing care to patients in their homes and community, promotes independence, prevents hospital admission, and supports timely discharge.
Main Duties / Responsibilities
* Manage a community nursing caseload independently, prioritising and coordinating care for patients with complex health needs.
* Deliver high‑quality clinical care, including assessment, care planning, treatment, and evaluation for wound care, medication management, long‑term conditions, palliative care, and catheter care.
* Provide clinical leadership and decision‑making support for complex cases, ensuring safe, effective, and patient‑centred care.
* Supervise and support junior staff – Band 5 nurses, nursing associates, healthcare assistants and students – through delegation, mentorship and professional development.
* Work collaboratively with multidisciplinary teams (GPs, therapists, social care, hospitals, specialist services) to coordinate holistic care and prevent unnecessary admission.
* Maintain accurate clinical documentation and ensure compliance with Trust policies, safeguarding procedures, infection control and professional standards.
* Contribute to service improvement and quality assurance, including audits, incident reporting and implementing best practice.
* Promote patient independence and wellbeing, supporting individuals and carers to manage health conditions and remain safely at home.
* Driving – holding a full UK driving licence and access to a car (car‑driver essential).
Key Responsibilities
To lead, coordinate and manage an allocated caseload of community nursing patients within Waltham Forest ICT North, delivering high‑quality care and supporting multidisciplinary care teams while safeguarding clinical and administrative standards.
Qualifications and Experience
* Registered Nurse (RGN) with current effective status on the Nursing and Midwifery Council (NMC).
* Post‑registration experience, including leadership or management of staff.
* Experience in community nursing, specialist care and working with complex physical, psychological and social needs.
* Ability to handle staff conflict and difficult situations.
Benefits and Working Conditions
High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) – 15 % of the basic salary (minimum £4 714 to maximum £5 941 p.a. pro rata for part‑time).
Probationary period – standard for external applicants; internal applicants exempt unless currently subject to a probationary period.
