Job Summary

Part‑time – 22.5 hours per week. This role is for a Community Staff Nurse within the Wakefield Tuberculosis Team in the South West Yorkshire Teaching Partnership NHS Foundation Trust.

Under the guidance of a Specialist TB Nurse, you will work with patients across the community and in their homes, providing person‑centred care and supporting the team in health assessments, prescribed and supportive care.

The position is based in Wakefield with close links to all relevant hospitals, community agencies and the Health Security Agency.

The service goal is to reduce the risk of new TB diagnoses and to provide high‑quality treatment and care for all people with TB.


Duties and Responsibilities

* Visit adults, children and families to support active or latent TB patients at home across Wakefield, including those with multi‑ or extensively drug‑resistant TB.
* Deliver TB screening clinics, using phlebotomy skills for QuantiFERON testing in accordance with Trust clinical standards and the NMC Code of Conduct.
* Record all patient contacts and activities accurately and contemporaneously using SystmOne.
* Participate in health promotion activities across health, social care, third‑sector organisations and other relevant organisations.
* Support and facilitate direct or video observation therapy for patients undergoing prescribed TB treatment, in line with TB management guidelines.
* Travel timely across Wakefield to undertake home visits and deliver clinics.
* Participate in large health screening programmes and disease management of patients under the direction of senior nurses.
* Carry out BCG vaccinations in clinic where appropriate.
* Liaise with regional TB teams to support and obtain clinical supervision.
* Deputise for the Specialist Nurse in MDT clinics and meetings as required.
* Assist with management of the caseload in the absence of senior nurses.
* Perform health assessments and prescribed care.
* Participate in clinical sessions delegated by senior nursing staff.
* Participate in training of clinical staff and other professionals.
* Participate in development of the service under supervision of senior nurses.

Person Specification

Physical Attributes
* Ability to undertake duties and demands of the post; satisfactory sickness record over the previous two years.
* Able to fulfil Occupational Health requirements for the post (reasonable adjustments where necessary).
Training
* Appropriate clinical skills.
Experience
* Experience working in community settings.
* Substantial secondary care experience.
* Evidence of relevant clinical skills.
* Ability to work as part of a team.
* Evidence of motivation and enthusiasm.
* Ability to work on own initiative and make decisions as required.
Personal Attributes
* Responsive attitude and approach.
* Dress appropriately for the environment.
* A current driving licence and access to a car during the working day is essential (reasonable adjustments considered for unsuitable drivers).
Qualifications
* Registered Nurse.
* Evidence of meeting revalidation requirements.

* Understanding of government legislation/documentation affecting the NHS.
* Evidence of health promotion skills.
* Awareness of issues facing people from different cultural backgrounds.
* Demonstrate innovative practice.
* Demonstrate motivational interviewing skills.

* Track record of reliability and good attendance.
* Ability to work flexibly, including occasional out‑of‑hours work.
