We are looking to recruit permanently to a Senior Practitioner role based within the Inner City & East Community adult social care team.
This is a great opportunity to develop your leadership and supervisory skills working in the inner city which is a diverse area of Bristol. It is an exciting time to be part of a community social work team as we work towards a new locality model of service delivery.
You will support practitioners to;
* Work with adults from a variety of groups including physical disabilities, mental health, learning disabilities, older adults and carers.
* Undertake Care Act assessments and planned and unplanned reviews.
* Undertake s42 (Care Act) safeguarding enquiries.
* Maximise use of social capital, reablement support and technology enabled care to undertake support conversations, reassessment where required and update support plans.
* Support & advise practitioners in weekly ward rounds/bed management meetings; CPA meetings, Blue Light Meetings and any other family and/or multi-disciplinary meetings including Continuing Health Care assessments.
The work in all teams can be pressured & challenging, so a creative approach is necessary, as is the ability to be confident with the '3 Tier' model to support the team to find strengths-based solutions, including innovative technological solutions.
You will have support from the Management Team and Service Manager. You must have a keen interest in and experience of working in a multi-disciplinary way, be able to work quickly and proportionately and have confidence around your decision making and positive risk taking.
The Senior Practitioner role involves supervising social care practitioners and social workers overseeing case management and supporting the development of ASYEs & student social workers.
You will be part of the leadership team and undertaking strategic work and building relationships with internal and external partners.
You will need to be a good communicator, with excellent time management skills.
