Overview

The post holder will work collaboratively with Luton Borough Council, ELFT Adult Mental Health Service and CAMHS. The post holder will be attached to the Luton Community Mental Health Service working directly with Care Leavers (18-21 years, with the view to 18-25yr after the post has been embedded).

The post holder will be a highly specialist and experienced Nurse with substantial knowledge in mental health, including child and adolescent mental health (CAMHS). The role involves working with Luton CAMHS, Luton Borough Council, ICB colleagues and other professionals to support young people’s physical, emotional and mental health needs in preparation for transition into adulthood and independence, from statutory, voluntary or school-based sectors such as NHS trust, local authority, school or charitable organisations.


Main Duties

1. 1.1 Work directly with Care Leavers in the holistic assessment of health needs, supporting transition into adult services, sign posting and referral onto appropriate agencies as indicated.
2. 1.2 The role requires effective communication with young people, including those who have previously been hard to reach. A patient, enabling and empowering approach is required to help young people meet their own health needs.
3. 1.3 Work directly with those young people identified as having an emotional health need, providing one-to-one support and referring or sign posting to specialist services as required.
4. 1.4 Provide regular health and well-being advice at the Luton Care Leavers Hub, including face-to-face sessions and engagement via approved social media technology (video / WhatsApp calls / messaging) to ensure those placed out of borough can access health advice and support.
5. 1.5 Support Care Leavers to make behaviour changes leading to positive lifestyle changes.
6. 1.6 Work with local services to meet the health needs of Care Leavers with special educational needs and disabilities to ensure adequate provision when accessing services.
7. 1.7 Support Unaccompanied Asylum Seeking Children to access specific trauma-based therapies as appropriate and consider health and wellbeing needs to prepare for independent living.
8. 1.8 Attendance at London Regional Looked After Children Forum.
9. 1.9 Link with Luton Borough Council Care Leavers Forum / Participation Team.
10. 1.11 To be a resource of specialist advice relating to care-experienced teenagers to colleagues within CAMHS, other professionals and agencies/organisations working in adolescent mental health.
11. 1.12 To liaise with and, where appropriate, participate in joint work with Clinical Nurse Specialists in Youth Justice, Justice and Substance Misuse services and with the Trust adolescent and adult inpatient service.
12. 1.13 To liaise with inpatient services in the adult mental health directorate and adolescent inpatient services where appropriate.
13. 1.14 To ensure that the needs of the client group are paramount, and to adhere to local Child Protection Procedures and Guidance (2019) and the local Multi-Agency Adult and Safeguarding Policy and Procedures (2019).
14. 1.15 To ensure acceptable safety levels regarding verbal and physical aggression from more complex clients, to maintain safety for self and others, and to adhere to the lone worker policy and update training in breakaway techniques when necessary.
15. 1.16 Requirement to travel to young people placed both in and out of borough, using public transport or own car in line with ELFT travel and expenses policy.
16. 1.17 Link with Luton Virtual School to support young people’ access to education, training or employment.
