Bedded Units Medicines Optimisation Pharmacist – Band 7

Are you looking for a rewarding career within the pharmacy world? Perhaps a move from secondary care but still wanting to work as a clinical pharmacist and help patients with their medicines? If yes, this could be the post for you.

An exciting opportunity has arisen for a dynamic, motivated and innovative pharmacist to join the expanding Medicines Management team at Central London Community Healthcare Trust.

We are looking for a full‑time pharmacist to provide a clinical pharmacy service at our bedded units within South West Hertfordshire & North London.

As an integral member of the team, you will provide specialist expertise in the use of medicines whilst supporting the public health and social care needs of patients. You will be involved in developing and delivering a high‑quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Discharge to Assess wards) covered by the CLCH Bedded Services team.

You will work with a team of Band 7 pharmacists and senior clinical pharmacists to gain new skills in each rotation, and to develop strategic, quality improvement and leadership capabilities.


Main Duties

* Provide a high‑quality specialist clinical pharmacy service across a range of inpatient settings in the Community Hospitals.
* Offer professional expertise, advice, support and training to health care professionals and patients within the Trust.
* Ensure individualised patient pharmaceutical care and clinical prioritisation on a day‑to‑day basis at ward level.
* Travel across the different sites where we have Bedded Services inpatients – requiring flexibility and a full, valid UK driving licence with access to a car for business purposes, unless you are exempt under the Equality Act 2010.


Education and Qualifications

* Vocational Masters Degree in Pharmacy.
* Professional registration with the General Pharmaceutical Council (GPhC).
* Postgraduate Clinical Pharmacy Certificate qualification or equivalent.
* Postgraduate Clinical Pharmacy Diploma qualification or equivalent.
* Independent prescriber status.


Experience

* Hospital pharmacy experience.
* Experience of providing a clinical pharmacy service with effective medicines management initiatives.
* Evidence of basic teaching and tutoring in a hospital setting.
* Experience working effectively within multi-disciplinary groups.
* Experience of structured medication review.
* Primary and community health experience.
* Experience of audit techniques.
* Experience of training.
* Experience of project or change management.


Skills and Knowledge

* Understanding of the NHS and its political environment.
* Sound knowledge of prescribing, pharmacy and medicines management issues.
* Proficient in Medicines Reconciliation on admission and discharge and Medicines Management processes.
* Good core therapeutic knowledge.
* Excellent verbal and written communication skills.
* Ability to manage own time and prioritise workload effectively.
* Computer literate – office applications, internet, email and electronic diary systems, and advanced spreadsheet skills.
* Adaptability and flexibility to meet service needs.
* Capability to think clearly and work under pressure.
* Well‑organised, self‑motivated with an enthusiastic approach to work.
* Effective negotiation skills and ability to persuade others.
* Knowledge of interface issues.

* Health cleared by Occupational Health for the role specified.

Background Checks

This post requires a Rehabilitation of Offenders Act (Exceptions Order) 1975 clearance and a Disclosure and Barring Service check to assess any previous criminal convictions.

£55,524 to £62,652 per annum (inclusive of HCAS).
