Job Introduction
Lincolnshire County Council has an exciting opportunity in for aSocial Worker Level 1position based within the Adult Care Team at Pilgrim Hospital, Boston.This role would be suitable for a newly qualified Social Worker requiring their ASYE or a Level 1 candidate looking for more experience in a hospital setting.
About the Role
You will be undertaking assessments of need both within hospitals and the community settings to ensure our customers are able to live independentlives and for those that need support, the right supportpackage, in the right place, at the right time.
The customer is at the centre of the process; our highest continuing aim is to provide the best service possible to service users and their families.
The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we supportare at the very centre of their own discharge.Working creatively when planning supportpackages, thinking outside of the box to ensure an individualcare plan is in place that empowers the service user to participate.
LCC is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to supportaccess to the right services, reducing delays and delivering benefits.
Work is undertaken face to face, on site, to continually offer the best service we can to those people who we support.
You will work as part of the 7-day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.

We are a team of Community Care Officers, Social Workers and other professionals, based within Pilgrim Hospital.We strive to be proactive and use a Strengths Based Approach, to enable our customers to return home and live as independentlyas possible.
We put the customer at the centre of the process and advocate their wishes to achieve the best possible outcomes. We are asupportiveteam and welcome the different strengths and experiences new people bring.
