About the role

As a Patient Service Coordinator in the Community Heart Failure Service, you will be a central figure in enabling seamless, patient-centred care for individuals dealing with heart failure. This non-clinical role is essential for enhancing access, reducing clinician burden, improving patient experience, and supporting service efficiency and quality.

This post is part time 30 hours per week.

The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.


Main duties of the job


Key Responsibilities

* Manage timely and appropriate patient bookings, reducing delays in care and ensuring patients are seen at the right time by the right clinician.
* Coordinate follow-ups, obtain diagnostic reports, and handle referrals, improving continuity of care and assist the clinical team in preventing readmissions or deterioration.

Person specification


Qualifications

* Evidence of sound literacy and numeracy skills
* Word processing training in Microsoft Word for Windows and other programmes
* NVQ Admin Level 3 in Administration or equivalent experience
* European Computer Driving Licence


Knowledge and Expertise

* Understanding of the need for confidentiality at all times
* Understanding of Equal Opportunities
* Understanding of how to create and structure office systems e.g. Emis Caseload stationary ordering
* Understanding the principles of PC file/document management
* Understanding of high levels of distress service users can present with and willingness to gain awareness of generic relevant past experiences
* Knowledge of NHS structures and processes


Experience

* Secretarial and administrative experience in an office environment and in remote working
* Worked as part of an administrative team
* Advanced keyboard skills
* Experience of taking minutes
* Experience of data entry
* Experienced in the use of Microsoft Office
* Experienced in arranging meetings and preparing the associated documentation
* Ability to manage, within the limits of an patient coordinator’s services role, service users’ distress both in person and on the phone
* Prior experience of working in a healthcare setting
* Wide experience of various database systems and their design
