The Unplanned Community Nursing Team reflects an integrated approach designed to meet the evolving needs of the local population, with a special emphasis on proactive community care. The team provides a 7‑day, 24‑hour service, reacting promptly to patients’ needs as they arise.

As the unplanned care team, we cover the entire borough of Doncaster, delivering all aspects of nursing care to patients in their own homes. We foster a supportive environment with opportunities for career development, a strong in‑house training programme, links with partner universities, and a comprehensive 12‑month preceptorship for new staff.

We believe in care that treats the whole person, not just the condition, enabling patients to stay at home longer and leave hospitals sooner. Community nurses work closely with the wider multi‑disciplinary team, including GPs, local hospice, Clinical Lead Nurses, and specialist nurses.

We are seeking a Staff Nurse who demonstrates vision and skill to work innovatively, flexibly, and in partnership with other agencies to support the MDT team and deliver care to the high standards we aim for.

Main duties of the job

This role involves assessing, planning, implementing, and evaluating nursing care in patients’ own homes and over the telephone. The post holder will apply excellent communication, IT, and clinical skills, telephonic assessment, diplomacy, and sound judgement to manage referrals into Community Nursing effectively and engage relevant services within appropriate timeframes.

• Setting, maintaining and delivering high standards of care in varied locations.
• Under the supervision of a Senior Nurse, triage incoming referrals to assess patient needs and determine care priority.
• Adopt a multisystem approach to case management, coordinating with case managers and the wider health and social care community to ensure a seamless patient journey across Doncaster.
• First‑line management of junior staff, offering support, mentorship, and acting as a positive role model while ensuring standards and best‑practice evidence are upheld.

Qualifications

• Registered General Nurse
• NMC Registered

Experience

• Experience working with professionals from other disciplines.
• Experience working as part of a team and ability to work autonomously.
• Evidence of effective team working.
• Community Nursing Experience.

• Able to provide evidence‑based care responsive to patient needs.
• Able to use own initiative and make decisions.

Knowledge

• Demonstrable effective clinical skills.
• Ability to move and handle patients in accordance with the Organisation Manual Handling policy and sufficient mobility for a community role.
