The Unplanned Community Nursing Team reflects an integrated approach designed to meet the evolving needs of the local population, with a special emphasis on proactive community care. The team provides a 7‑day, 24‑hour service, reacting promptly to patients’ needs as they arise.
As the unplanned care team, we cover the entire borough of Doncaster, delivering all aspects of nursing care to patients in their own homes. We foster a supportive environment with opportunities for career development, a strong in‑house training programme, links with partner universities, and a comprehensive 12‑month preceptorship for new staff.
We believe in care that treats the whole person, not just the condition, enabling patients to stay at home longer and leave hospitals sooner. Community nurses work closely with the wider multi‑disciplinary team, including GPs, local hospice, Clinical Lead Nurses, and specialist nurses.
We are seeking a Staff Nurse who demonstrates vision and skill to work innovatively, flexibly, and in partnership with other agencies to support the MDT team and deliver care to the high standards we aim for.
This role involves assessing, planning, implementing, and evaluating nursing care in patients’ own homes and over the telephone. The post holder will apply excellent communication, IT, and clinical skills, telephonic assessment, diplomacy, and sound judgement to manage referrals into Community Nursing effectively and engage relevant services within appropriate timeframes.
