The main duties of this role are to support within the Acute illness clinic, diagnosing and treating patients with minor illnesses.
About us
Swallowfield Medical Practice is made up of 8 GPs with a range of experience that enhances the service offered. 3 Paramedic Practitioners see all acute illnesses in an on-the-day clinic. A nursing team of 6, including two phlebotomists.
Under our PCN, along with the paramedics we have a Pharmacist, Social Prescriber, Mental Health Practitioner and MSK first point physio. We are supported by a large team of receptionists and administrators who ensure the wheels keep turning behind the scenes. We have a dispensary with a team of 8 people and provide medications to patients that don’t have another pharmacy within 1 mile of their home. We strive to put patient care first and ensure a safe running practice in a fun and friendly environment for both staff and patients.

Purpose of Job:

Main Responsibilities:
Diagnosing and Treating Patients Presenting with Minor Illness
* Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
* Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
* Ensure clinical practice is safe and effective and remains within boundaries of competence, and acknowledge limitations.
* Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary.
* Work from the surgery and within communities as an autonomous practitioner caring for patients and families.
* Work as an autonomous practitioner in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.
* Undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiating investigations where appropriate.
* Formally and informally impart knowledge and skills to colleagues, promoting peer review and best practice within the work environment.
* Communicate at all levels within the team to ensure an effective service is delivered.
* Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
* Ensure evidence-based care is delivered to high standards and high quality patient care.
* Work with local policies and procedures.
* Enhance own performance through Continuous Professional Development, imparting knowledge and behaviours to meet service needs.

* Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc., and perform investigatory procedures requested by GPs.
Administration and professional responsibilities
* Participate in administrative and professional responsibilities of the practice team.
* Ensure accurate and legible notes of all consultations and treatments recorded in patient notes.
* Keep the clinical computer system up to date with accurate details.
* Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.
* Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
* Maintain collection and maintenance of statistical information required for regular and ad hoc reports and audits.
* Attend and participate in practice meetings as required.
* Training, Professional and Personal Development
* Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individual’s responsibility to remain up to date with recent developments.
* Participate in the education and training of students of all disciplines and the introduction of practice staff where appropriate.
* Maintain continued education by attendance at courses and study days as deemed useful for professional development, ensuring PREP requirements are met.
* If necessary, expand the role to include additional responsibilities with full training provided.
* Develop and maintain a Personal Learning Plan.
* Work closely with nursing, reception and office teams to ensure smooth practice operations and clear role understanding.
* Establish and maintain good liaison with other surgeries and agencies, including secondary care. Attend and contribute to practice meetings as requested. The only reason for not attending will be annual, study or sick leave.

Person Specification
Qualifications
* Qualified Paramedic registered with the HCPC
Experience
* Minimum of 5 years experience as a Paramedic; evidence of continuing professional development; proven ability to evaluate safety and effectiveness of own practice.
* Skills in education and training.
