We are recruiting on behalf of our client, Home Instead, an award-winning home care provider consistently recognised for delivering outstanding quality care. They are the first London home care business to be rated Outstanding by the Care Quality Commission three times in a row.
Due to continued growth and demand, they are now looking to appoint a dedicated and experienced Client Care Manager to join their compassionate and professional team in New Malden.

The Role
As Client Care Manager, you will be responsible for managing your own portfolio of clients and ensuring they receive safe, person-centred care of the highest standard.
Key duties include:
* Managing client portfolios, ensuring compliance with company policies, CQC regulations, and best practice standards.
* Conducting risk assessments, care planning, service reviews, quality assurance visits, and MAR chart updates.
* Overseeing hospital discharges and liaising with multi-disciplinary teams to ensure smooth and safe transitions home.
* Building strong working relationships with healthcare professionals, such as GPs, district nurses, pharmacies, and social workers.
* Supporting families with professional guidance and reassurance.
* Working closely with the internal team to match Care Professionals appropriately to client needs.
* Promoting the highest standards of service delivery with a focus on compassion and person-centred care.
Candidate Requirements
* At least 2 years of experience in a Care Management role.
* Strong knowledge of CQC regulations and KLOEs.
* Proven experience writing care plans and risk assessments.
* Good IT skills, including Outlook, Excel, and experience with care management systems.
* Minimum of 5 GCSEs (A-C), including Maths and English.
* Must live locally to New Malden, hold a driving licence, and have access to a vehicle.
* Excellent communication, organisational, and time management skills.
* Professional, caring, and positive approach to work.
What's on Offer
* Salary: £34,500 - £38,000 per annum (depending on experience).
* Hours: 40 per week, Monday-Friday (flexibility required to meet the needs of a 24/7 care service).
* Contract: Permanent, with a 6-month probation period.
* Pension: Auto-enrolment scheme.
* Expenses: All reasonable out-of-pocket expenses covered, plus mileage at 45p per mile.
