GP Primary Choice Ltd are looking for a specialist social prescriber to join their team to cover GP surgeries in the Colchester area on a part-time basis for 29.5 hours per week, working across 4 days.
The position will be responsible for providing advice and support to the public in a holistic manner linking their needs to services, as available in the area in which they live. It will specifically incorporate working with patients experiencing dementia, their families and carers. The successful post-holder will be required to drive due to travel required throughout the Colchester area.
Main duties of the job
* Contribute to reducing the overall future longer-term statutory health and social care demand through a proactive and holistic care approach.
* Pro-actively search for patients who would benefit from this service from a variety of risk registers in place.
* Work closely with care and residential homes to provide advice and support to both patients and their families.
* Work closely with the local hospital for patients who are ready to be discharged back into the community.
* Be able to have the aptitude to act independently, make home visits, and have the competency to make judgements on complex information.
* Listen to service users, carers and family members and be capable of dealing with distressing situations. To be able to give emotional support if required.
* Provide advice, support, information and assistance in obtaining services from other agencies and where appropriate, make initial contacts on the Service Users behalf and act as advocate if needed.
* Discuss patient problems with GPs or their primary care teams updating the practice database after each visit.
Experience
* Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups.
* Driving license and access to one’s own transport.
* Strong listening and communication skills.
* Familiarity with local resources and services and how to access them.
* Motivated by helping people, with care and empathy.
* Ability to deliver person-centred support in a non-judgmental way.
* Experience of completing holistic, person-centred care-planning assessments.
* Understanding of the needs of small, volunteer-led community groups and the ability to support their development.
* Commitment to collaborative working with all local agencies.
* Experience and confidence in the delivery of lifestyle change interventions.
