This role is within Wakefield CAMHS ReACH Team (Crisis). We are looking for Band 5 Clinicians (RN/SW/OT) to join our ReACH (Crisis) Team. If you are looking to make a real difference and keep children/young people at the heart of what you do, a career with us may be just for you.

CAMHS ReACH Team is a developing service that supports young people, families and carers during crises across settings including hospitals, homes, schools and clinics. The team provides 365-day cover on long shifts (8am–8pm) on a rotating schedule.

Job Description

Main duties of the job

* You will manage your own caseload, providing high-quality care appropriate to the young person’s needs, including risk assessment and management.
* You will triage new assessments and be responsible for taking calls from young people and families in crisis.
* You will develop strong working relationships with professionals across the district; excellent communication and engagement skills are essential.
* You will contribute to service development within the clinical governance structure and Trust policies, adhering to NHS guidance from statutory bodies.
* This is a fast-paced role requiring a proactive attitude towards tasks.
* The team provides outreach to those who are difficult to engage, delivering intensive support to meet identified needs, contributing to a reduction in admissions to inpatient provision.

Job Responsibilities

JOB PURPOSE: The post holder will be responsible for assessment, planning, implementation and evaluation of skilled interventions for allocated clients on a defined caseload, working within a multi-disciplinary CAMHS team.

* Provide comprehensive assessment of children, young people and their carers, planning and delivering interventions to address identified needs.
* Apply knowledge of current issues and legislation (e.g., Children Act, Mental Health Act, Care Programme Approach).
* Contribute to service development within governance structures and policies; adhere to NHS guidance and statutory registrations. Promote safeguarding and welfare of children and young people.

Professional/Clinical Skills

* Assess needs, develop plans in partnership with the young person and carers, implement care plans and review effectiveness.
* Competent in risk management and care within the Care Programme Approach framework.
* Collaborate with clients, carers and other agencies to meet holistic needs.
* Manage enquiries and direct referrals per Operational Policy; participate in in-hours Single Point of Access rota.
* Maintain professional registration and ongoing professional development; manage medicines safely where applicable.

Person Specification

Essential

* Ability to work as part of a multidisciplinary team; good communication skills; ability to travel around the Trust; flexible working including extended hours; reliable and enthusiastic; professional attitude.
* Skills in assessing and managing mental health needs and risk in children/young people; experience in MDT settings; ability to deliver therapeutic interventions in one-to-one or group formats; experience collaborating with multiple agencies.

Desirable

* Ability to support colleagues and supervise students/trainees; holistic assessment of emotional and mental health needs with coexisting factors.

Qualifications

* Essential: Registered Mental Health Nurse / Social Worker / Occupational Therapist.
* Desirable: Mentorship qualification or equivalent; progression toward practice educator role.

Special Knowledge/Skills

* Working knowledge of government legislation (Mental Health Act, Care Act, Safeguarding); clinical governance; IT literacy; good written and verbal communication; knowledge of audit processes and carer services.
