* Hours: 37 hours per week
* Shift pattern: Mixed – 9‑5 pm and 12‑8 pm shifts over 7 days
* This post is permanent
Responsibilities
* Provide intensive home treatment and care intervention for individuals with complex mental health, physical and social care needs who present in crisis, as an alternative to hospital admission.
* Support early discharge from hospital by participating in discharge planning and immediate follow‑up from acute in‑service user care.
* Assess individuals during pass‑through from acute in‑service user care.
* Exercise clinical leadership of junior team members, prioritising workload, supervising and providing regular appraisal and clinical supervision.
* Ensure that the practice of junior members is within legal and ethical frameworks, appropriate codes of professional conduct, mental health legislation and contractual obligations.
* Assist the Team Leader in the rapid deployment of crisis resources in response to service user need.
* Interface between the Community Mental Health Team and in‑service user services, and other mental health network services and agencies.
