St John's Medical Centre has an exciting opportunity for two Advanced Nurse Practitionersto join our enthusiastic and supportive Practice Team. The successful candidates must hold prescribing qualifications, be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.
St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 14,000 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review.
We are flexible with regards to the number of hours for the roles but would be looking for hours per week,working Monday- Friday between the hours of 08:00am-18:30pm. We offer an extended hours clinic on a Monday evening between 18:30pm-20:00pm, there may be a requirement to cover this shift.
Main duties of the job
Working as an Independent Practitioner, the successful candidates will be required to assess, diagnose and manage patients with a variety of conditions. We offer a same day service and their maybe a requirement for occasional home visits to be undertaken.
The main duties of the role include but are not limited to:
1. Being able to conduct a comprehensive physical health assessment of patients with complex multiple healthcare needs.
2. Interpret results of multiple different assessments and investigations to make a diagnosis, and plan to deliver care.
3. Prescribe and work with patients to manage their medicines, and to be able to make onward referrals where required.
4. Competently make ethical, evidenced decisions and interventions when faced with complexity and assess the risk associated with these decisions
5. Comfortable working both independently but also as part of a multi-disciplinary team.
6. Comfortable with IT, our clinical systems include SystmOne, Accurx and AskmyGP. (Full training and support of these systems can be provided).
7. To work as Duty Buddy on selected days to help the Duty Doctor with the management of tasks, admin and results.
About us
St John's Medical Centre has a friendly and welcoming team. In addition, we are a Training Practice and regularly welcome GP Registers and Medical Students. Currently we have 3 GP Registrars and a Student Nurse on placement with us.
Our Clinical Team consists of 3 Partners, 2 Salaried GP's, 2 Clinical Pharmacists, 2 Advanced Nurse Practitioners, 3 Practice Nurses, 2 Health Care Assistants, a Diabetes Nurse and our Home Visiting Team. Amongst these team members, there are special interests in Diabetes, Respiratory Disease, Gastroenterology and Sexual Health.
In addition to our clinical team, we have our Reception, Admin and Management Teams that are vital in the daily running of the Practice and supporting our Clinicians delivery of excellent patient care.

Advanced Nurse Practitioner
Primary Responsibilities
15. Working autonomously in an independent and Advanced Nursing role
16. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
17. Implement and evaluate individual specialised treatment plans for chronic disease patients
18. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
19. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
20. Undertake the collection of pathological specimens
21. Process and interpret pathology and other test results as required
22. Maintain accurate clinical records in conjunction with extant legislation using primary care software
23. Ensure read codes are used effectively
24. Maintain chronic disease registers
25. Prioritise health issues and intervene appropriately
26. Support the team in dealing with clinical emergencies
27. Recognise, assess and refer patients presenting with mental health needs
28. Extended and supplementary prescriber, adhering to extant guidance
29. Support patients in the use of their prescribed medicines, reviewing annually as required
30. Contribute to practice targets (QOF etc.), complying with local and regional guidance
31. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
32. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
33. Support the clinical team with all safeguarding matters, in accordance with local and national policies
34. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
35. Deliver opportunistic health promotion where appropriate
36. Participate in the administrative and professional responsibilities of the Practice Team
37. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients care record
38. Ensure accurate completion of all necessary documentation associated with patient healthcare and registration of the practice
39. Ensure collection and maintenance of statistical information required for regular adhoc reports and audits
40. Ensuring when significant events are identified, they are recorded appropriately on GP Team Net.
Secondary Responsibilities
In addition to the primary responsibilities, the advanced nurse practitioner may be requested to:
