Orchard Surgery is looking for an experienced, enthusiastic and motivated person to join our friendly practice team. The ideal candidate will already have at least 2 years of primary care experience as a Practice Nurse.
We are looking for an exceptional nurse with excellent communication and clinical skills, who will work closely with our Nurse Lead and nursing team to provide high quality patient care. We are looking for a nurse who has the ability to care for patients with flexibility and empathy. We offer regular clinical supervision and training to support your development.
We have 5 GPs, 4 paramedics, 3 clinical pharmacists, 2 pharmacy techs, a First Contact Practitioner physiotherapist, social prescriber, care coordinator and an extensive admin and management team.
We are one of 2 practices in Park and Orchard Primary Care Network in Horsham. Our management team are actively involved in the running of the PCN and our GP managing partner is one of two PCN co-clinical directors. Our vision for our populations is closely aligned and our focus is on improving the care of our frail elderly housebound population and improving access and quality of women's health care in Horsham. The practice has recently been awarded funding to develop and establish a Women's Health Hub. We are working closely with community organisations in both these arenas.
Main duties of the job
* Identify,diagnose and manage treatment plans for patients at risk of developing along-term condition, as appropriate
* Workwith other health care professionals to diagnose, monitor, manage & treatlong-term conditions including non-drug-based treatment methods using amanagement plan, and in line with national and local policies and practiceneeds
* Infectioncontrol policies, training and implementation
* Reviewmedication for therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice, national and practice protocols with GP support
* Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare
* Supportpatients to adopt health promotion strategies that promote healthy living, andapply principles of self-care, utilising community assets such as the HorshamWellbeing Hub
* Demonstratesensitive communication styles to ensure patients are fully informed andconsent to treatment

Job responsibilities
* Provideassessment, screening and treatment services
* Providehealth education advice to patients
* Participatein organising and coordinating the provision of nursing services for thepractice to include the delegation of duties to other practice team members asappropriate
* Providenursing treatments to patients in participation with general practitioners orindependently, adhering to agreed protocols
* Providegeneral and specific health screenings to the practice patients with referralto general practitioners as necessary
* Advisepatients on general health care and minor ailments with referral to GPs asnecessary
* Delivervaccination and immunisation as required
* Provideappropriate wound care
* Suppliesand equipment treatment rooms and other areas
* Ensureclaims are logged with the practice manager
* Ensurethe maintenance of equipment and stock levels in clinical rooms
* Pathologicalspecimens and investigatory procedures
* Undertake to support the administrative team inthe collection, storage and dispatch of pathological specimens including swabs,smears etc.
Administrative and professional responsibilities
* Participatein the administrative and professional responsibilities of the Practice team
* Ensureaccurate notes of all consultations and treatments are recorded in the patient notesand on the clinical computer system as appropriate
* Ensureaccurate completion of all necessary documentation associated with patienthealth care
* Ensurecollection and maintenance of statistical information required for regular andad hoc reports, audits and research programmes
* Attendand participate in practice clinical meetings on a regular basis

Research projects and training
