Summary:
We are seeking a dedicated Care Practitioner to join our team in Woodbridge and surrounding areas. This permanent, full-time role is essential in providing high-quality care and support to our clients. The Care Practitioner will play a vital role in enhancing the well‑being of individuals, ensuring they receive compassionate and professional assistance in their daily activities. This position is crucial for maintaining the standards of care within our medical and nursing environment.
Responsibilities:
* Provide personal care and support to patients, including assistance with daily living activities.
* Monitor and record patients' health and progress, reporting any changes to the healthcare team.
* Administer medications as prescribed and ensure compliance with care plans.
* Assist in the development and implementation of individual care plans.
* Maintain a clean and safe environment for patients, adhering to health and safety regulations.
* Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.
* Participate in training and development opportunities to enhance skills and knowledge.
Requirements:
* A relevant qualification in health and social care (e.g., NVQ Level 2 or equivalent).
* Previous experience in a Carer role, preferably in domiciliary care.
* Strong communication and interpersonal skills.
* Ability to work effectively as part of a team and independently.
* Compassionate and patient‑focused approach to care.
* Valid UK driving licence and access to a vehicle.
* Shifts are a combination of 8am-2pm and 3pm-9pm
* Pay per shift (rather than per client visit)
* Mileage paid between client visits at a rate of 45p per mile
