Job Overview:
Facilitates the coordination of services for developmentally delayed children who qualify for Early Intervention. Assists child and family in developing the Individualized Family Service Plan. Provides supportive counseling and crisis intervention to child and family. Advocates for children on state and local levels.
Qualifications:
Required:
* One or more of the following acceptable:
o BA in Psychology, Teacher of the Handicapped, Social Worker, or BSN
o Master's degree in social work (LSW) from an accredited program and one-year experience in public health or with children who are chronically ill or disabled
o A NJ licensed nurse with a bachelor’s degree in Nursing (BSN) or Bachelor's degree in Social Work (BSW)
* Certification/Licensure as dictated by profession
* Ability to work independently with a demanding client caseload
* Ability to provide supportive counseling and crisis intervention
* Exhibits empathy and cultural/ethnic sensitivity to a diverse client population
* Effective problem-solving abilities
Certifications and Licenses Required:
* Certification/Licensure as dictated by profession
o Teacher of the Handicapped Teaching certification
o Bachelor of Social Work (BSW) certification
Scheduling Requirements:
* Full Time, 40 hours/week
* Ability to work remotely
Essential Functions:
* Establishes working relationships with children who are developmentally delayed and their families
* Coordinates assessments in accordance with Early Intervention/DOH guidelines
* Assesses children's service needs across various areas: social/emotional, developmental, familial, medical, cultural, economic, educational, etc.
* Develops and reviews the Individualized Family Services Plan (IFSP) for eligible Part C children, ensuring proper notifications
* Facilitates coordination of services for Part C children and families through linkage to service providers and EI programs
* Promotes communication among service providers and advocates for appropriate services
* Provides supportive counseling and crisis intervention for Part C children and families
* Assists families with transition planning to preschool, special education, or other services at age three
* Manages electronic files, including data entry into EIMS/DOH database
* Supports the Part C Unit in the absence of the Unit Coordinator
* Participates in policy development, evaluation, and revision for the Unit
* May travel by car for meetings and home visits
* Job-specific contacts include children and families, regional and state EI professionals, and resource/referral networks
