As a Senior Nurse Care Manager (RNLD)/Senior Social Worker (LD Care Management and Transitions Service), you’ll thrive on enabling adults with learning disabilities to realise their potential and have a voice in decisions about their care and support.

Our Nurse Care Managers/Senior Social Workers use a person-centred approach that values the uniqueness and strengths of individuals and involves their carers, family and wider community. This role supports adults with learning disabilities in Leeds within a growing adult social care team.

About You

You will be qualified as:

* A Registered Learning Disability Nurse and registered with the Nursing and Midwifery Council or Senior Social Worker (registered with SW England).
* An Approved Mentor/Practice Assessor
* Practice Educator Level 2 or equivalent, or undertake this on appointment (funded by us)

You will bring to the role:

* a minimum of 2 years’ post qualifying experience
* knowledge and understanding of current learning disability practice and the role of a multi-disciplinary team, including statutory and voluntary agencies
* skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities
* ability to build good working relationships with professional colleagues and partners, chair meetings and represent the council in a multi-agency environment and engage positively with individuals, families and carers
* experience of using a person-centred approach, creativity in problem solving and reflective practice techniques
* ability to lead, motivate, mentor and supervise staff, with a commitment to continuing professional development of self and others


About The Role

As a Senior Nurse Care Manager/Senior Social Worker, you will be central to a network of partners working across health and social care in Leeds who are committed to increasing integration for better services supporting people with learning disabilities.

You will manage a complex and diverse caseload to safeguard the welfare of individuals, families and carers. You will assess needs and manage risk, devise and implement support plans, commission services and evaluate outcomes, working with colleagues and partners to provide direct support to individuals and their carers.

You won’t be doing this alone, you will be supported and guided by team managers, and in your turn play a key role in mentoring and advising other team members. Job Description

Job Purpose

With support from Team Manager, provide a high quality and comprehensive service to learning disabled adults, their carers and community in accordance with organisational needs, policy, practices and within statutory guidelines. At all times carry out responsibilities within the Nursing and Midwifery Code of Conduct or Social Work England.


Responsibilities

* Manage a caseload of higher levels of challenge and complexity, including assessment and management of risk, advising less experienced team members on casework.
* Carry out specific pieces of work on cases allocated to other team members.
* Utilise professional skills to provide direct support to individuals and their carers.
* Undertake assessment, support planning, monitoring, reviewing, commissioning services and contributing to service development using a strength-based approach.
* Maintain and update case notes/records and provide reports for legal contexts and court evidence if required.
* Contribute to safeguarding and promotion of welfare, including multi-agency work where needed.
* Develop and maintain effective working relationships with professionals internally and externally to ensure integrated care management.
* Lead, mentor and supervise staff and contribute to staff development and supervision of student placements.
* Chair multi-agency meetings, determine plans and contribute to capacity building within the community.
* Contribute to planning and developing future services and policies to promote innovative support.


Qualifications

Appropriate professional qualification (Registered LD nurse or Social Worker) and registered with the NMC or SW England on commencement. Supported Learning in Practice/Approved Mentor/Practice Assessor Programme are essential on appointment.


Essential Requirements

* Able to effectively manage, prioritise, review and evaluate service and support plans with service users.
* Able to work in a team with organisational and time management skills to meet deadlines.
* Strong communication skills for reports and working with partners, service users and carers.
* Able to negotiate, challenge and manage conflict and exercise professional judgement.
* Experience using a person-centred approach and leading complex caseloads.
* Ability to supervise and develop staff and students.
* Knowledge of relevant legislation, policies and safeguarding for vulnerable people.
* Experience in a statutory/health setting (minimum 2 years).


Desirable

* Possession of a full current driving licence.
* Knowledge of consolidating learning for specialist practice.
* Knowledge of services available to users, families and carers.
