The Cancer Services Team are looking for Cancer Follow-up Coordinators. The Cancer Follow-Up Coordinators will work alongside Cancer Support Workers and Clinical Nurse Specialists along with the wider Cancer team to ensure patients have a timely, efficient, effective and supported follow-up period following completion of their cancer treatment. Post-holders will be based in a specific cancer-type team and work closely with the clinical and non-clinical members of that team.
Responsibilities
* Track patients following treatment, including enrolment onto and discharge from remote follow-up systems.
* Coordinate investigations and symptom assessment questionnaires as per a pre-defined schedule.
* Evaluate results within a clinically supervised protocol.
* Communicate with patients the outcome of review via standardised letters.
* Escalate to the clinical team as indicated by the protocol.
