Clinical Nurse Specialist/Clinical Paramedic Specialist - Hospice Outreach Service
Join a committed multidisciplinary team delivering compassionate palliative care to people with life-limiting illnesses. The CNS/CPS roles are equivalent to Band 7 and operate within the Hospice Outreach Service in the community, covering a seven-day-a-week rota with telephone support, clinic, and home-based assessments.
We care for patients with a range of conditions (life-limiting illness, heart failure, respiratory disease, frailty, neurological conditions, and oncology) with a focus on quality of life and patient-centered goals. The service collaborates with other community teams to optimise patient experience and development.
Location: Maidstone Road, Pembury, Tunbridge Wells, Kent, TN2 4TA. Band 7, Salary £47,302 to £54,129 per year. Permanent, Full-time.

Responsibilities
* Coordinate and deliver high-quality care to patients with terminal illness in their homes, community hospitals, and care/nursing homes.
* Provide clinical advice and interventional support to other professionals on palliative care issues; help patients achieve personal goals and preferred place of care.
* Deliver Hospice advice and support to more people; participate in seven-day service including weekends and bank holidays; contribute to duty planning.
* Engage in multidisciplinary team planning to ensure patients are seen in the right place at the right time.
* Attend GP practice palliative care meetings; promote End of Life Care tools; support information provision to patients and families; tailor advice to individual needs.
* Perform advanced physical assessment ensuring holistic evaluation; maintain high standards of symptom control and End of Life Care documentation.
* Ensure patient reviews at MDT or caseload reviews in line with NICE guidance.
* Support risk awareness and safety for patients and self; contribute to policy development and guideline updates; mentor juniors and volunteers.
* Advise and support the multi-disciplinary team on pain and symptom control, advanced care planning, psychosocial care, and resource use.
* Engage in audit, performance reporting, and research activities in palliative care; develop mechanisms for auditing care and implement improvements.
* Provide training and development opportunities; supervise/reflective practice; promote education to staff and care homes on End of Life Care; support external CNS/CPS development.
Qualifications
Essential
* Registered Nurse (Level 1) or Registered Paramedic (HCPC).
* Minimum 3 years post-registration experience.
* Evidence of ongoing Professional Development.
* Relevant BSc; Principles of End of Life Care masters module.
Desirable
* Ability to motivate and inspire; initiative and flexibility; effective delegation.
* Ability to develop rapport with diverse people; credibility, tact, and diplomacy.
* High computer literacy, including mobile technology.
* Analytical thinking, proactive in decision making and risk mitigation; strong organisational skills.
General and Other
Maintain professionalism per Codes (NMC, GMC, HPC, HCPC). Participate in Hospice annual review; use ICT competently; undertake mandatory training and external training to enhance the CNS/CPS role. All other duties reasonably requested; strive for excellence.
