Lincolnshire Partnership NHS Foundation Trust (LPFT) offers a range of services to support people with learning disabilities in the community. LPFT's adult learning disability service provides specialist health support to people with learning disabilities who require assessment and/or treatment for their physical and mental health, including support with behaviours of concern. The adult learning disability service operates from three multi-disciplinary hubs across Lincolnshire and delivers a county-wide community home assessment and treatment team.

* Validate GP registers, deliver training, and cover annual leave and sickness of colleagues as required.
* Develop and motivate others, build positive working relationships within the adult learning disability service and with partner agencies, and demonstrate excellent interpersonal and communication skills.
* Demonstrate enthusiasm and commitment to high quality care and continuous quality improvement; undertake training relevant to the post.
* Promote the rights of people with a learning disability to access physical health care, make reasonable adjustments, and apply and train others in the principles of the Mental Capacity Act.
* Engage mainly in community settings with some acute liaison as required.
* Address inequalities experienced by people with a learning disability within the healthcare system.

Candidate Profile / Qualifications

* Registered practitioner (Band 5-6 progression) with current registration and the ability to provide liaison support for people with learning disabilities.
* Clear commitment to high quality care, continuous quality improvement, and strong initiative with excellent team-working ability.
* Progressive mindset with the ability to develop and motivate others and build positive relationships with the service and partner agencies.
* Willingness to undertake training relevant to the post; strong interpersonal and communication skills; and a practical understanding of the Mental Capacity Act and its application.
* Although predominantly community-based, some acute liaison experience is desirable.
