The Wallingford, Henley and Thame AMHT have a rare opportunity for you to become part of the well-established function, that is part of the broader South Oxfordshire Adult Mental Health Team (AMHT)?

The South AMHT consists of four bases in Wallingford, Abingdon, Didcot and Wantage and consists of a multi-disciplinary team of a range of different clinical staff members. We also work closely with embedded third sector services in each locality.

You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and Occupational Therapists and hold your development as central. We offer regular supervision and you will be encouraged and supported to undertake any training relevant to your professional development.

The role is 37.5 hours per week, the role will require some weekends working (one weekend in six) and we have the opportunity for staff to work late and early shifts across the week. We welcome flexible working so please call us to discuss how we may be able to accommodate other working patterns. You will not be required to work night shifts.

The role will require travel so it is essential that the candidate has a valid UK driving licence and access to a car.


Main duties of the job

The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of an multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs.

The responsibilities include:

* Working as an autonomous practitioner an active participant of the multi-disciplinary team undertaking specialist nursing assessments in collaboration with the service user and their families/carers and presenting back to the wider MDT.
* Working in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.
* Deliver evidence-based interventions following NICE guidelines to promote recovery and relapse prevention in a variety of settings.
* Regularly review care and outcomes, supporting service users to complete clinical outcome measures and monitoring progress and adapting treatment options as required.

* Professional Qualification (Degree or equivalent)
* Evidence of Continuing Professional Development (CPD) portfolio
* Professional Registration


Desirable criteria

* Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)


Occupational Experience


Essential criteria

* To have experience of working within a multidisciplinary team
* To have highly extensive experience which should include community and working with mental health
* To have a good knowledge and interest in community care and have specific clinical expertise in mental health


Skills required - IT / Admin


Essential criteria

* To be IT literate
* To have excellent English written communication skills
