Overview
An interesting and varied vacancy has become available for a qualified Band 4 Associate Nurse Practitioner. The role will be in the Community Mental Health Team function within the wider Older Adult integrated Mental Health services.
Responsibilities
* Oversee and monitor your own caseload and support senior staff in delivering quality care and treatment programmes to patients with mental health challenges. Supervision will be provided to support you in the role.
* Undertake a range of other generic clinical duties to support the team functions and be responsive to the needs of patients, their family and carers.
* Work within a MDT to support older adult mental health service users in the community to remain at home and prevent hospital admission.
